and anomalies of fetus in utero is also increased, maternal weight
alone was not associated with an increase in congenital
anomalies.
Diabetes was significantly associated
with the increase in the rate of anomalies seen in our
population. Identification of maternal weight as a risk
factor in epidemiologic studies may be a surrogate for
pregestational diabetes.

Therefore, it is observed that a representative tissue biopsy (when approachable) and radiological findings are good method of diagnosis of abdominal tuberculosis. Strongly suggestive clinical features with positive non specific investigation findings are also an indication for anti tubercular treatment
Treatment of abdominal tuberculosis is in no way different from that of other conventional Anti tuberculous treatment.

Define" Labour" vs "delivery"
There is a difference between these two terms "labour" and "delivery "
Labour: Term is used when strenuous process is involved in expulsion of fetus through vagina
Delivery : Term is used when the fetus is extracted from either abdominal route or vaginal route with or with out strenuous work by pregnant.
Delivery is term used in broad sense
Labour is used in certain condition.
labour term can be included in delivery but delivery term should not be included in labour in all conditions.In other words all labour process can be come under delivery but all deliveries can not be put under labour.

కామెర్ల పేషెంట్ కథ ..A STORY OF JAUNDICE PATIENT.
అసలు దేవుడు వున్నాడా?
ప్రతి రోజు నాకు ఇదే డౌట్ .రోజు రోజుకి పెరిగి పోతుంది . వుంటే కొందరికి అన్యాయం,ధనికులకి సౌభాగ్యమా?
లేబర్ రూం ఇన్ ఛార్జ్ గా వున్న నాకు రోజు రోజుకి హృదయాన్ని ద్రవించే సంఘటనలు కనిపించటం పెరుగుతూనే వున్నాయి.
ప్రమాదకరమైన జబ్బులు పేదవారికి ,ధనికులకి సమానం గానే ఇచ్చిన దేవుడు డబ్బు మాత్రం సమానం గా పంపిణి చేయలేదు .పస్తుతం లేబర్ రూం లో ఒక వైరల్ కామెర్లు ( హెపటిటిస్ -బి ) రోగి వుంది .ఆమె పేరు దుర్గాబాయి ( పేరు మార్చబడింది ).వయసు 20 .వూరు జంగారెడ్డి గూడెం దగ్గర వున్న ఒక పల్లెటూరు .కాయా కష్టం చేసి రోజు వారి కూలి డబ్బులుతో బ్రతికే బక్కజీవులు .
దుర్గాబాయి కి పెళ్లి ఐయింది .ఎనిమిది నెలల గర్బవతి గా వున్నప్పుడు (కడుపు లోనే) బిడ్డ చనిపోయింది .డెలివరీ కష్టం కావడం తో సిజేరియన్ ఆపరేషన్ చేసి కడుపులో చనిపోయిని బేబీ ని బయటికి తీయవలిసి వచ్చింది .కుటుంబమంతా దుఃఖ సాగరం లో మునిగారు .
రెండవ సారి గర్బం దాల్చిన దుర్గాబాయి ని చూసి కుటుంబం ఆనందించే లోగానే మరో ప్రమాదకరమైన సంగతి తెలిసింది .దుర్గాబాయి కి కామెర్ల జబ్బు వచ్చింది .అత్యంత దారుణమైన హెపటిటిస్ -బి వైరస్ వ్యాధి . తెలిసాక వెంటనే దగ్గరలోని గవర్నమెంట్ ఆసుపత్రికి తీసుకు వెళ్లారు .అక్కడ డాక్టర్ పోస్ట్ ఖాళీ.నర్సు తనవల్ల కాదని చెప్పి ప్రైవేటు హాస్పిటల్ కి లేదా విజయవాడ సిటీ గవర్నమెంట్ హాస్పిటల్ కి తీసుకు వెళ్ళమని సలహా ఇచ్చింది .ఖర్సులు కి భయపడి దూరమని దగ్గరలో ని ఏలూరు వెళ్లారు .అది డిస్ట్రిక్ట్ హాస్పిటల్ .కాని వ్యాధి నిపుణులు డాక్టర్స్ కొరత .చివరికి విజయవాడ రాక తప్పలేదు .
కేసు హిస్టరీ చదవ గానే అర్దమైంది .హై రిస్క్ కేసు .జనరల్ మెడిసిన్ / గాస్త్రో ఏంటి రోలజిస్ట్ , గైనకా లజిస్ట్ , స్కాన్నింగ్ డాక్టర్ , ల్యాబ్ డాక్టర్స్ , ఇలా చాలా మంది కావాలి .
ఈసారి ఆమె గర్భం నిలుస్తుందా?

Jaundice in Pregnancy-Most Common Cause -Viral Hepatitis-Complications -Management
- Viral hepatitis is most common in poor ,unhygienic , malnourished pregnant ladies
- 6 major types: Hepatitis -A,B,C,D,E,G
- All are RNA viruses except Hepatitis -B which is DNA virus
- Complications for mother : Increase severe bleeding immediately after delivery (PostPartumHaemorrhage ),Coma ,Blood Coagulopathy, Infection , Hepatorenal syndrome,Maternal death
- Complications to fetus: Abortions, Abnormal babies, Delivery of fetus with out completing full term , Fetal death in utero.
- Hospitalisation
- Bed rest
- Isolation of the infected patient
- High Calories ,Rich Carbohydrates food
- Proteins to be given adequate quantity
- Proper Hygiene ,Safe water ,Safe food is must
- .Disposable syringe/Gloves are advised for paramedical staff to prevent contamination , infection from patient to others
- Glucose , fruit juice ,tender coconut water, etc Liquids to be given plenty
- Antibiotics,those donot give overload on liver
- Fatty food may harm jaundice patient but not proved much
- Drugs which cause liver toxicity should be avoided
- lactulose-Osmatic laxative can be given daily to clear bowel fecal contest
- Mother to be taken Active Immunization at 0 day , 1month ,6 months -If mother is not infective clear cut way
- Immunoglobulins are safe in pregnancy.Hepatitis vaccines are also safe during pregnancy
- Regular Blood tests
- Blood /fresh frozen plasma
- Vit-K 5mg IM -Increase PT-Helpful
- No Role of Termination of pregnancy_canot change final result or mortality of mother.
- Inj Ergometrine Immediately after delivey of baby to prevent post partum haemorrhage.
- Baby of jaundice infected mother should be recived active and passive immunisation immediately after birth.
- No contra indication to breast feeding feeding
- All women should be recieved Hepatitis vaccine six months prior to pregnancy.
- Husband also has to be tested and treated or vaccinated for hepatitis

Why jaundice occur in pregnant women?
Causes of jaundice in pregnant lady can be grossly divided into two groups
1.Exclusively pregnancy related causes:
A) Hyperemesis ( Exessive vomiting during first 3 months period of pregnancy)
B) Hypertensive disorders of pregnancy like Preeclampsia ( High BP+ Albuminuria +_ Pedal edema) , Eclampsia ( Pre eclamsia + fits) , DIC ( Uncontrollable bleeding disease -High deathRates -Endotoxic shock)
C) Liver diseases like Acute fatty liver
D) Most Common - Intra partum choleastasis ( Pregnancy specific hepatic ,gall bladder ,Bile duct abnormality ,function, obstruction )
Second group of causes of jaundice in pregnancy which are unrelated to pregnancy are as follows
A) Most common causes : Viral hepatitis
B) Gallstone-Obstructive jaundice
C) Drugs -Isoniazide, Phenothiazine etc
D) Hemolyticjaundice-Mismatched blood transfusion ,

General advice for pregnant lady about sleep diet exercises
Most of the pregnant ladies are anxious about somany things.they eagerly waiting for a person to clear their doubts.Gynaecologists are usually busy with emergency cases and vexed to explain small things to all pregnants.Practically not possible to clear all doubts of all pregnants ,though getting information is right of a pregnant.Diet
- Balance diet to be taken.Extra calories are needed 350 kcal during pregnancy and 450 kcal during first six months of lactation.
- Better to take frequent meals.Instead of taking large meal ,it is advicable to divide the same quantity to take 2-3 times.Pregnancy causes gastrits ,indigestion .less quantity with frequent meals give less discomfort to pregnant and less strain on stomach.
- At least 2 -3 litres Water to be taken daily
- Protein rich diet like dals , soya , beans , nuts are good food
- Green leafy vegetables to be mixed with dal then it gives good taste and health
- Fat food is also needed but 30-50g per day quantity is enough
- Bcomplex food like germinating seeds are best not only for mother but also for baby health in utero
- Milk is rich source of calcium.It is whole food.250 ml milk to be taken at the time of breakfast and night supper.ICalcium is bone builder for baby.
- Iron utensils are best option to prepare food.It gives some extra iron to pregnant which finally prevents iron deficiency anemia .
- Pregnant should take atleast 10 hours sleep.8hrs night sleep and 2 hrs afternoon.
- Those who suffers lack of sleep should not use sleeping pills .These can have warm milk which gives good sleep.
- Exercises like walking is safe in pregnancy in all trimesters
- Some particular yogasana( not all ) can do upto 7-8 months period of gestation.
- Should not participate in running race or competetive sports.
- Keeping breaks in between work is enough.Can attend thier duties upto 8months
- Reading comic books , Listening pleasant music are encourajable activities.These help in good sleep.

- ఆకు కూరలు చాల మంచివి. ప్రతి రోజు తింటే గర్భిని కి పోషక విలువలు అన్ని అందుతాయి .
- మలభద్దకం , రక్త హీనత , కడుపులో మంట తగ్గిస్తాయి .
- పాల కూర , బచలి కూర , తోట కూర మొదలైనవే కాక కొతిమీర , మెంతి కూర కూడా వాడటం మంచిది .
- కాడకూరలు ఉత్తమమైనవి .చిక్కుళ్ళు , కంది ,మినుములు ,పెసలు , రాగులు, సజ్జలు ,జొన్నలు ,భటానీలు, శెనగలు మొదలైనవి.
